Motor Accident

The following documents are required:

  • Duly completed claim form

  • Repairs estimate from a certified mechanics

If the Vehicle is confirmed a write-off, then these documents are required:

  • Original purchase invoice

  • Original vehicle license

  • Original certificate of insurance

  • Ignition and other lock keys

  • Photograph of the damaged vehicle

Motor Fire/Theft

The following documents are required:

  • Duly completed claim form

  • Original Purchase Invoice

  • Original Vehicle License

  • Original Certificate of insurance

  • Ignition and other lock keys

Universal Insurance HomePlus

The following documents are required:

  • Duly completed claim form

  • Witness statement/Security report (if any)

  • Purchase receipts/Invoice of stolen items

  • Quotation for the repairs of damage parts from a certified building contractor

  • Police Report (this is applicable if the claim is a Burglary case)

  • Fire brigade report (in fire claims)

  • Photograph of damaged properties or premises

Corporate Business Claims

All Risks Insurance, Burglary and Goods in Transit

The following documents are required:

  • Duly completed claim form (All Risks), claim form (Burglary), claim form (Goods-in-transit)

  • Police report

  • Statement of claim (detailing the lost or damaged items with value)

Fire Insurance

The following documents are required:

  • Duly completed claim form

  • Statement of claim (detailing the lost or damaged items with value)

Fidelity Guarantee

The following documents are required:

  • Duly completed claim form (All Risks), claim form (Burglary), claim form (Goods-in-transit)

  • Police report

  • The culprits’ letter (s) of employment

  • Name of the culprit(s)

  • Statement of claim (detailing how much was stolen and by who)

Personal Accident, Workmen Compensation and Group Personal Accident ( Injury Cases)

The following documents are required:

  • Duly completed claim form (Personal Accident), claim form (Workmen Compensation), claim form (Group Personal Accident)

  • Medical report

  • 3 months pay slips preceding injury

Personal Accident, Workmen Compensation and Group Personal Accident (Demise Cases)

The following documents are required:

  • Duly completed claim form

  • Medical report

  • 3 months pay slip preceding demise

  • Burial certificate

  • Certificate of cause of demise
