  • Quotation for the repairs of damage parts a certified building contractor,
  • Police Report (this is applicable if the claim is a Burglary case),
  • Fire brigade report (in fire claims),
  • Photograph of damaged properties or premises.

  • Duly completed claim form,
  • Statement of claim (detailing the lost or damaged items with value).

  • Statement of claim (detailing how much was stolen and by who),
  • Name of the culprit(s),
  • The culprits' letter (s) of employment,
  • Police report.

 

Personal Accident, Workmen Compensation and Group Personal Accident ( Injury Cases)
